The forms you need to complete the application process are PDF links below. Please use this checklist to ensure that all forms (items 1-7 below) are returned to the Admissions Office. If you have any questions, please call us at (415) 258-1905 Monday through Friday between 8:30 a.m. and 4:00 p.m. or email admissions@sandomenico.org.
- APPLICATION FORM
- STUDENT APPLICATION: We are interested in learning more about you and the activities in which you participate. Please submit your own work in your own handwriting. If you submit the Common Application Form, you will need to request a copy of the San Domenico Student Statement.
- APPLICATION FEE: The completed application form should be returned to the Admissions Office with a non-refundable check payable to San Domenico School, in the amount of $100.00.
- TRANSCRIPT RELEASE: This form should be signed by your parent/guardian and given to your school registrar/secretary who will mail your transcript directly to San Domenico. This form should not be returned to our office.
- PRINCIPAL/COUNSELOR RECOMMENDATION: This recommendation form, along with a return envelope, should be given to your principal, head of school, or counselor.
- MATH AND ENGLISH TEACHER RECOMMENDATIONS: These two recommendation forms, along with return envelopes, should be given to your Math and English teachers.
- PHOTO: Please include a recent student photograph (optional).
CAMPUS VISIT: Please call the Admissions Office to schedule a visit. You and your parents will have the opportunity to sit in on classes, have a tour of campus and take part in student activities.
TESTING: Scores from one of these tests below are required:
- SSAT (our code is 7084)
- ISEE (our code is 054074)
- HSPT (High School Placement Test)
- Eleventh grade applicants need to take the PSAT (Preliminary School Admission Test).
TUITION ASSISTANCE: Online Tuition Assistance Applications were due January 14, 2010.
